Social Services

Community Case Managers in Fan Free Clinic’s Social Services Department work with clients to empower them to secure resources such as substance abuse services, safe and affordable housing, and medical treatment. Clients work to develop a plan that will allow them to meet their goals and tackle barriers one by one.

Available services may include:

  • Coordination of access to medical, social, educational, legal, dental, employment and other community services
  • Assistance accessing social security, unemployment, Medicaid, Food Stamps or other public assistance programs
  • Mental health counseling & support groups
  • Social activities such as a monthly bingo night
  • Transportation assistance for medical appointments
  • Assistance identifying and securing housing
  • Complementary therapies (e.g., acupuncture, reflexology)
  • Housing stabilization assistance
  • Referrals to job training and money management services
  • Food pantry and nutrition services
  • Education specifically targeted toward people living with HIV/AIDS, including medications adherence counseling
  • Referrals to infectious disease and general medical providers, substance use/abuse services, clothing and household furnishings
